Coaching Agreement

Confidentiality. All information obtained in the course of the professional service is confidential unless there is a compelling reason for its disclosure. Your coach will disclose confidential information without a specific release if it is necessary to prevent foreseeable imminent harm to the client or another. In all circumstances, coaches will be judicious in the amount of information that is disclosed. Coaches may disclose confidential information without the consent of the client only as mandated or permitted by law.

When possible, coaches inform clients about the disclosure of confidential information and possible ramifications before the disclosure is made. Coaches will disclose confidential information to third parties only with the appropriate written consent. Coaches must disclose certain confidential information as required by law or if the failure to disclose such information would put the client or others at risk of harm or compromise their well-being.

You understand that your coach will protect your information as confidential unless you state otherwise in writing. If you report child or elder abuse or neglect, or threaten harm to yourself or someone else, you understand that necessary actions will be taken and your confidentiality agreement is limited in this regard. Furthermore, if your Coach is ordered by a court to provide information or testify, he or she will do so to the extent the law requires.

Electronic Communications. You understand that the confidentiality of electronic communications (EC) cannot be guaranteed. EC may include, but are not limited to, email, text messages, online chats, Skype and other videoconferencing, and voicemail. EC are inherently vulnerable and insecure and may result in the unintentional harmful disclosure of of personal information. You acknowledge that you implicitly consent to Dr. van Scoyoc sending EC to you by: (a) the inclusion of your email address on Dr. van Scoyoc’s forms, (b) providing your email address in session, (c) your sending an email to Dr. van Scoyoc, or (d) the disclosure of your email address in any text message, chat session, Skype conversation, or other kinds of EC. You further acknowledge and understand that Dr. van Scoyoc may not read or respond to EC until the next scheduled appointment and that you will not use EC for emergencies.
